What to Expect:

You’ll meet with a Care Manager who understands the system and is here to help you navigate it. We’ll check in regularly, plan ahead with you, and jump in fast when problems pop up.

This service is free for Medi-Cal members and available $60/hour for others. Reach out to us to see if we’re a fit for you.

Alex James

Alex (she/her) serves as the Care Manager at HH & Co. She has more than six years of experience providing case management and has worked with many different demographics throughout Santa Cruz County. Alex brings a person-centered approach to case management and believes that empowerment and collaboration in care leads to better outcomes and quality of life for individuals. She received her bachelor’s degree from California State University of Monterey Bay in Collaborative Health and Human Services.

Alex Olivas

Alex (they/them) serves as a Care Manager at HH & Co. They have experience providing case management services in community-based settings and supporting individuals navigating a range of needs. They are committed to creating a welcoming and inclusive environment where individuals feel heard and supported. Alex is able to provide services to Spanish-speaking clients.

Alex earned their Bachelor of Arts in Psychology from California State University, Monterey Bay.
